ABOUT HOW WE WORK

We work with a variety of professionals, including Integrated Care Boards (ICBs), Local Authorities, Case Managers, and Healthcare Professionals, to ensure that you receive the best possible care by the best possible people.

Our senior clinical team has over 20 years of experience in the healthcare industry. They lead a fully comprehensive audit structure to ensure clinical governance is maintained throughout the service at all times. This means that we are constantly reviewing our processes and procedures to ensure that we are providing the highest quality of care to our service users.

Our Process

We aim to provide full care mobilisation within 14 days of initial commission, which includes clinical assessment, comprehensive care plan development, funding agreement and contracts in place.

Assessment: We will assess your needs and create a care plan that is tailored to your specific requirements.
Funding Agreement: We will work with you to secure funding for your care, whether this is private, ICB approved, or a PHB allocated.
Care Plan Development: We will work with you to develop a comprehensive care plan that outlines your needs and the services that we will provide.
Selection of Carers: You will have the opportunity to meet and select carers who are qualified and experienced in providing the care that you need.
Monitoring and Reviews: We will monitor your care, care requirements and provide regular reviews to ensure that your needs are being met.
App and Monthly Reports: We will use our app to provide you with regular updates on your care. We will also provide you with monthly reports that outline the services that we have provided.
Clinical lead visits: Our clinical lead will visit you every regularly to ensure that your care is meeting your needs.
